Pick up at the library:
- Forms and instructions, including Maine 1040 forms, can be printed off for $0.15 per page. Please know exactly what forms you need; library staff cannot provide advice.
Receive in the mail:
- To request Maine State Tax Forms in the mail, visit Maine Revenue Service's Electronic Request Form or call Maine Revenue Service at 207-624-8475.
- To Request IRS Tax Forms in the mail, visit the Forms and Publications by U.S. Mail page on the IRS website or call the IRS at 800-829-3676.
